Check Charm Factory on the internet and I googled "jewelry tags" and found several sources for personalized tags and Microstamp makes the stamp so you can stamp your own tags. There was one listing where there was a one-time fee of only $25.00 for typesetting your logo to be able to stamp your tags for you.

Jewelry supply closeouts.com has a 25% off if you pay only $1.00; yes $1.00 and they have things like that I think. Anyway if they don't they certainly have good bargains.
Then there is jewelry supply.com, they have displays... and the tags you mentioned in 2 different styles. They are a little more expensive though.
